Transition to Adult Healthcare for People with Developmental Disabilities

The Boggs Center on Developmental Disabilities, through the Foundation for Healthcare Advancement, received a one year grant from the Special Hope Foundation. The Boggs Center is using the funding to convene “The NJ Developmental Disabilities Transition to Adult Healthcare Forum.” The forum brings together stakeholders from across New Jersey to focus on building the capacity of the adult health care system to support the transition of youth and young adults with developmental disabilities to age and developmentally appropriate health care. This project shifts the paradigm in transition from pediatrics being ready to “send” patients, to readying the adult health care system to “receive” patients. The outcome of this planning will be the development and dissemination of an “Action Blueprint for Transition to Adult Health Care” for New Jersey, an important clinical and policy tool in making the adult health care system responsive to adults with developmental disabilities.

The Forum planning process is unique in bringing together diverse stakeholders representing all levels of the adult health care system: individual patients and families, primary and specialty practitioners, MCOs and providers, Medicaid as a payer, and state agencies as regulators and providers, in concert with the medical education system and advocates, to focus on transition and adult health care for people with developmental disabilities. The Forum brings together those who have or can identify needs, with payers and providers who can innovate and redesign practice.

Deborah M. Spitalnik, PhD, Professor of Pediatrics and Executive Director of The Boggs Center serves as principal investigator and co-chair of the Forum. Alfred F. Tallia, MD, MPH, Professor and Chair of the Department of Family Medicine and Community Health, serves as co-chair of the Forum. Caroline Coffield, PhD, Instructor of Pediatrics and Interdisciplinary Training Coordinator at The Boggs Center serves as Project Manager and Susan Ellien, MSW, Policy and Dissemination Coordinator at The Boggs Center, serves as Project Coordinator.
